Do the Benefits of Glaucoma Surgery Outweigh the Risks?

As with any surgical procedure there are potential risks that need to be weighed against the benefits, Glaucoma is no different.  Even the most advanced cases of Glaucoma can be treated, patients need to collaborate with their eye surgeon to ensure the benefits offset the risks.
Glaucoma is a disease that is associated with aging.  It is a disease of the eye that causes advanced decline of the optic nerve.  The optic nerve which is at the back of the eye carries the visual message from the retina to your brain. Glaucoma is caused by increased pressure within the eye that doesn’t provide an environment for the nerve’s health.

In order to treat Glaucoma, the IOP (intraocular pressure) must be lowered.  Lowering the intraocular pressure prevents (or slows) the damage to the optic nerve.  Treating Glaucoma begins with the use of medication.  If this fails to treat or your eyes do not tolerate the treatment your ophthalmologists will often turn to surgical procedures.

How Exactly Is My Cataract Going to Be Removed?

Cataracts have become a common complaint amongst the aging population.  In fact, cataract surgery is one of the most common surgeries performed in the United States today.  Thankfully, surgery is very successful step in treating cataracts and improving patients’ vision.  The word surgery itself seems to invoke a lot of fear let alone pairing it with the word eye. The good news is that most patients undergoing cataract surgery not only experience improved vision, but they do so with little to no side effects. 

Cataract surgery is performed by making two incredibly small incisions in the cornea. The cornea is a transparent dome that is made up of tissue that covers the front of your eye.  The incisions that are made into this tissue are less than 3 millimeters.  This is very small incision. 

Once the two incisions have been made, your cataract surgeon will inject a gooey thick material into the front of your eye to help it keep its natural shape while the surgery is performed.  The goo that is injected is a substance that naturally occurs in your body.  The thickness of the bi-product ensures that leakage does not occur while the surgery is performed.

Phacoemulsification is the technical name of the cataract surgery we are discussing.  In Phacoemulsification your surgeon creates a lens capsule to hold the lens in place.  This is done by creating an opening with in the natural sac and then separating the lens from the capsule by using a salt solution.  Once a surgeon has opened the capsule the lens can move freely within it. A specialized ultrasound is used to break up the lens into a bunch of small pieces and suck it out of the eye.
Before Phacoemulsification, cataract surgery looked a lot different.  In prior surgeries cataracts were removed by making a large cut in the cornea and removing the lens in one solid piece.  This procedure required an extended recovery time as well as increased risk of infection, complication,and lasting side effects. 

In Phacoemulsification once the lens pieces are removed, more goo is injected into the lens capsule to help keep it open and to make room for the placement of the new lens.  The new lens is then placed into the lens capsule and allowed to unfold naturally.  The goo material will have kept the shape of the eye throughout surgery and the two small incisions will self-heal.

• Flap Complications: In order to reach the cornea a flap is made in the outer layer of the eye. Recovery After Lasik: What to Expect

LASIK is a common surgical procedure that is performed everyday throughout the United States.  The biggest concern that most patients have surrounds recovery.  It is important to note that although the surgery last only a few minutes per eye, the long-term recovery process, complete healing, can take upwards of six months.  Here are some common side effects and how you can relieve them.

Overall Discomfort and Minor Pain

Most LASIK patients describe a certain level of discomfort during and after laser vision surgery.  The most common sensations are light sensitivity, burning, or ripping.  Some patients say they barely notice these symptoms while others find them to be quite bothersome.  Symptoms can vary a great deal between what LASIK procedures and technique was used.  This minor pain and overall discomfort should dissipate in about two to four hours after the surgery.

Redness

Redness is a normal side effect of laser vision surgery and recovery.  Some patients experience dark red spots in the whites of their eyes for a couple of days after their surgery.  These spots are very similar to bruising and can take weeks to completely disappear.

Dry Eyes

One of the most common symptoms after LASIK is dry eyes.  More than fifty percent of patients express their need for eye drops after surgery.  Surgeons will recommend you purchase eye drops before the surgery in addition to the prescription eye drops that are prescribed to prevent infection and inflammation.

After the procedure you will be required to meet with your surgeon the next day for a post-operative appointment.  During this appointment you will have your vision checked and the ophthalmologist will check to make sure that your corneas are healing.  This is the time to address any symptoms you are experiencing with your doctor.  You will have several appointments throughout the healing process to verify that they process is occurring properly.  The intervals for these appointments will be one month, three months, and six months.  At the sixth month visit your eyes should be completely healed and your vision stabilized.

After surgery, to avoid complications, there are a number of activities that should be avoided including from wearing make-up to avoiding strenuous activities.  It is also important to avoid getting soap and water in your eye and you should avoid swimming or any activity in which you submerse your head in water.   Computer use should also be avoided or used cautiously.

Recovering from Cataract Surgery

Recovering from cataract surgery should be relatively short and unexciting especially when your health is good and there aren’t any other major issues with your eyes.  Based on statistics, once a cataract has been removed your chances of an excellent outcome and sharper vision.
Cataract surgeries without complications most often takes under ten minutes from start to finish.  Right after surgery patients will rest in a recovery room until the affects of sedation and anesthesia wear off.  Recovery usually lasts anywhere between a half hour to an hour. You will not be able to see clearly right away so it is important to have someone available to drive you home after cataract surgery.  Patients are given a pair of sunglasses for the ride home to protect their eyes from the sunshine and light glare.

Once patients are at home they are advised to take it easy for the rest of the day.  Your surgeon will tell when you can take off the protective shield that has been placed over your eye.  This is usually possible after several hours however, it is important to follow the instructions that you are given for your unique situation.  Remember that you will need to use the protective shield when you are sleeping for protection while your eye is recovering from surgery.  This will need to be done for several days. 

Recovery Time

Your vision will appear cloudy and distorted for awhile once you have removed the shield from your eye.  Your vision can take some time to adjust after the removal of a cataract.  Once your eye adapts to the intraocular lens that has replaced your natural lens the cloudiness in your eye will disappear. However, during this process some patients mention that their vision is a bit wavy and distorted.  If you experience this, it is important to note that it should last only about an hour.

After cataract surgery you may notice that your eyes are redder and bloodshot.  This is temporary because of damage to the blood vessels on the surface of the white part of your eye that can occur during surgery.  As your eye heals from cataract surgery this redness will disappear.
Many cataract patients report that their vision is clear within hours after surgery.  This however, varies from patient to patient.  Each person heals at a different speed.  It is crucial that you do not rush your recovery.  Even though some patients see clearly within hours of surgery some take as long as a week or two before they see sharp images.

Cataract patients will most often see their surgeon the day after their surgery to look for signs of complications.  After this period, it will be up to each patient individually to contact their surgeon if they have concerns or don’t feel the improvement in their vision is occurring how they thought it would.

Some people report dry, scratchy eyes right after cataract surgery however, as your eye begins to heal this will slowly fade.  About a month after cataract surgery patients are considered recovered and their eyes should be completely healed.

These Bad Habits Can Harm Your Eyes

Can Surgery Fix an Astigmatism?

During the last few decades a variety of methods to treat an astigmatism have grown widely accepted.  An astigmatism is a defect in your eye or in the lens that is caused by an abnormality on the spherical shape of your eye which distorts images. The Do's and Don’ts of LASIK Eye Surgery

Getting the best result out of your laser eye surgery occurs when you take the necessary steps to prepare for before and after the procedure.   As with any surgery, there are certain risks involved.  This is just one of the reasons it is important that you do everything possible to prevent issues from arising.  Below you will find some basic tips that can help assure you get the best results possible from your LASIK procedure. 

Laser Vision Correction Precautionary Steps

Even before LASIK is scheduled there are certain things that you can do to adjust your routine in order to receive the best results from your laser eye surgery.

• If you wear contact lenses, it is best to stop wearing them for a few weeks leading up to the surgery. Contact lenses distort the shape of the cornea which can cause the results from your LASIK procedure to be less than perfect. 
• Do not use any type of eye drops for at least a week before your laser vision correction surgery.

• Use protective eye wear especially sunglasses when leaving the house. The sun can damage even the healthiest of eyes.  Unhealed eyes are especially sensitive.  Sunglasses also help to eliminate debris such as sand and other dirt particles from entering your eye.

What to Expect After LASIK?

Laser vision correction is a surgical procedure used to reshape the cornea with an excimer laser. This type of surgery can be used on patients experiencing short sight, long sight or astigmatism. Laser vision correction is not an ideal solution for everybody and some people are not candidates for this type of surgery because of the severity of their problem or because of another medical issue. The best laser vision results mean that glasses and contact lenses will no longer be needed by the patient.

The Process of LASIK

One of the most common types of laser vision correction is LASIK or Laser-Assisted in Situ Keratomileusis. In a typical LASIK procedure, the patient will have numbing eye drops placed in the eye and then a lid holder applied to keep the eye open. A microkeratome is used to create a flap in the cornea, which is then bent back to expose the inner corneal tissue. The laser sends pulses of light to remove parts of the cornea while the patient focuses on another light to keep the eye as still as possible. The actual laser exposure time is usually less than a minute.

After the laser treatment, the corneal flap is repositioned. No stitches are needed because the flap will hold itself down when repositioned and heal itself. The patient is given an eye shield and eye drops and told to come back within a day or two, so the doctor can see if the eye is healing and whether there are any problems. The eye has great healing powers and it would be of great comfort to know that all corneal procedures, even complete transplants, do heal ultimately.

What to expect after the LASIK eye surgery

The post LASIK eye surgery period is just as important as the surgery itself. The cornea should completely heal. This takes time and requires a lot of aftercare, which should be done meticulously.

The average healing period after the LASIK eye surgery is approximately six months. During this period any vision problems that you may have been experiencing should have completely disappeared or at least considerably lessened. Complete healing of the corneal flap could take a bit longer, sometimes up to two years. But this should not give you any cause for discomfort or concern. Healing of the cornea is an ongoing process that does not or should not interfere with your vision or your daily life.

Usually there are a minimum of five follow-up visits are scheduled for most patients, post LASIK eye surgery. The first visit will most likely be within 24 hours of the surgery. It is of utmost importance to keep all these visits so that the doctor can keep track of the ongoing healing process and rectify any problems that may arise as you go along.

After LASIK eye surgery, it is natural to expect a little discomfort in your eyes, though you should not feel any pain. Part of the aftercare involves using eye drops to help alleviate this discomfort. This will have to be done for a minimum period of two to four weeks after the surgery.

Other common reactions that are common after undergoing LASIK eye surgery are a slightly blurred vision, feeling of dryness in the eyes, minor swelling and redness of the eye, sensitivity to bright light and watery eyes accompanied by a runny nose.

Tips to make this period more comfortable:

• To get over the minimal discomfort that comes along with LASIK you can take any over the counter pain medication that does not contain aspirin such as Tylenol or Ibuprofen.
• Give your eyes sufficient protection against sunlight or any bright light by wearing sunglasses when going out in the day.
• Give your eyes as much rest as possible by getting some extra shut-eye for at least two to three days post-surgery.
• The eye mask that was given to you needs to be worn immediately after the surgery for a minimum two to three hours and then during the next three to four days, it should be worn while sleeping.
• During the first two weeks, all water sports, as well as all water-related activities such as whirlpools or hot tubs, should be avoided. Scuba diving should be avoided for a minimum of six weeks.
• Your risk factor and the possible need for post-surgery rectifications depend on various factors, the most important one is the condition of your vision pre-surgery. If your vision had deteriorated there are higher chances that there will be some degree of complication in your surgery and you would be dissatisfied with the outcome. Also, if a small degree of astigmatism was present, the results could turn out to be unsatisfactory. Touch-up surgery is common and is normally covered by the initial cost, but the additional surgery is still an unpleasant thought.
• As with any other surgery, there are no absolute guarantees with LASIK eye surgery too. Complete success is determined by many factors acting together, most important of which is each individual healing powers.
• There are a few factors that act as a deterrent to anyone considering LASIK eye surgery. Anyone between 19 to 24 years of age is considered an unsuitable candidate for this procedure. Which Type of Laser Surgery is Best for You?

How does PRK laser eye surgery differ from LASIK?

PRK and LASIK vision correction methods are the most popular modern procedures for the correction of astigmatism, nearsightedness, and farsightedness. While their outcomes are ultimately similar, their procedures differ. Ophthalmologists usually determine the best procedure for a patient depending on the unique laser vision correction needs of the individual. But how does PRK laser eye surgery differ from LASIK?

PRK Laser Procedure

PRK (photorefractive keratectomy) Laser Procedure was the first eye surgery vision correction method – direct predecessor of the LASIK procedure. This procedure involves the removal of the thin outer layer before reshaping the corneal tissue using an excimer laser. Within a few days after the eye surgery, the epithelium repairs itself, growing back over the surface of the cornea.

LASIK Procedure

Lasik is a procedure which involves the creation of a thin flap on the cornea with the aid of a femtosecond or microkeratome laser. The thin flap is lifted in order to expose the corneal tissue, after which it is replaced following the reshaping of the cornea using an excimer laser.

PRK vs. LASIK

A major difference between PRK Laser surgery and LASIK is in the first part of the procedure. While LASIK involves the use of a thin flap at the corneal surface, PRK does not. Hence, so long as an individual is a candidate for LASIK, he/she would also be a candidate for PRK. However, a PRK patient may not necessarily be a LASIK patient. PRK is recommended for patients with thin or steep corneas, large pupils, irregular astigmatism, and history of eye syndrome or eye surgery.

The end results of both procedures are very similar – the difference is in the healing process. The initial recovery of PRK procedure is slow because it takes a few days for regeneration of new epithelial cells to take place so the surface of the eye will be covered. The risk of eye infection and vision haziness is also increased in PRK surgery in the few days after the eye surgery and the final outcome is gradual and could be achieved after several weeks. On the other hand, LASIK patients usually experience less discomfort with quicker stability of vision.

Although the healing process of PRK is a lot slower than LASIK, the procedure has some unique benefits. Since PRK surgery does not involve the creation of a thin corneal flap, applying treatment to the thick area of the underlying stroma is possible. This is especially beneficial to patients with thin cornea (who are not qualified for LASIK) and those who have previously undergone a LASIK eye surgery. PRK is also beneficial because the patient is not exposed to the risks of removing too much cornea or potential flap complications.

What is Myopia and What are the Correction Options?

Myopia, or nearsightedness, is the most common refractive error of the eye a situation where the eye does not refract light or bend properly to a single focus to view images clearly. In this case, close or near objects look clear while there is poor distance vision which causes distant objects to appear blurry. However, myopia is not an eye disease, but an eye focusing disorder.

Although myopia has a genetic link, its effect is usually more driven by environmental stress associated with near work such as computer use, reading, hand games, and a lack of quality outdoor time. In fact, an increasing cause of myopia is because of the adaptation of the eyes to prolonged periods of near work. Therefore, myopia is referred to as a “nearsighted” disorder one that impacts between 25-40 percent of the total U.S. population today.

Myopia – Correction Options

Rigid gas-permeable Contact Lenses

In comparison to soft lenses, rigid lenses are a lot smaller covering the pupil and slightly extending into the iris. Rigid lenses are made of a thin rigid plastic and they float on the eye tears like soft lenses, while oxygen passes through the lens to the eye surface. It may become necessary to clean the lenses when an eyelash or dust particle gets between the eye and the contact lens.

LASIK (Laser-assisted in situ keratomileusis) Surgery

Although contact lenses and eye glasses are myopia correction options, they are only temporary. Distant objects would still appear blurry if you are not using them.
